Arthroplasty Fixation

  • Cement

  • Porous coating

  • Press-fit

Cement

  • Especially useful in patients with osteoporosis
  • Cemented acetabular components have progressively increased rate of loosening over time, thus hybrid total hip replacement (cementless acetabular component with cemented femoral component) are frequently performed

Porous coating

  • Surface coating of metal, ceramic, or polymers, or combination.
  • Formed into beads, irregular particles, or etchings
  • Allows for bone ingrowth

Porous coated acetabular cup and proximal femoral components, polyethylene cup liner

Press fit

  • Acetabulum and femur prepared for maximum, optimal fit
  • Technically demanding surgery
  • Acetabular component frequently secured with screws
  • Femoral component may fit tightly, but may not fill canal in all planes
